Adult Housing Assistance
The Abraham Lincoln Centre delivers services to individuals who qualify for permanent supportive housing. Funded by the State of Illinois “Money Follows the Person” program, this service helps transition adults with a mental illness from nursing homes into an independent living arrangement.
Services
- Intense Case Management Services
- Assistance With Locating Housing
- Help With Community Living Skills (being a good neighborhood, understanding role as a tenant, etc.)
- Home Visitations
Eligibility Criteria
- Adults 18 years or older
- Have history of mental illness
- Previously or currently at-risk of homelessness
- Psychiatric plan is manageable
- Taking medications regularly, and no hospitalization within last year
- Ready to move back into community
- Must be referred by the State
